CompetencyAssessment/TrainingNeedsAssessmentandImplementationGuideGuideline:TG.001EffectiveDate:01June2000CompetencyCompaniontotheTechnicalResourceManualforHospitalTransfusionServicesTG.001Page1of61.0PrincipleImplementationofprocedures,whetherneworrevised,requiresplanningandorganization.Determiningtheplanofactionrequiresneedsassessmentandprioritizationofresources.2.0ScopeandRelatedPolicies2.1.Personnelshallreceiveinitialandcontinuedtrainingappropriatetotheirduties.Trainingprogramsshallbeavailableforthispurpose.9.3(A3.2.1),9.2(4.3.2.1;4.3.2.2)Theeffectivenessoftheprogramsshallbeassessedatleastannuallybyregularcompetencyevaluation,andassessmentofprogramelementsbymethodssuchasinterviewswithparticipants,reviewoftestresultsormonitoringofchangesinerrorrates.9.1(A3.2.1;A3.2.3),9.2(4.3.2.3;4.3.3.1)2.2.Thebloodbankortransfusionserviceshallhaveaprocesstoensuretheemploymentofadequatenumbersofqualified(byeducationandexperience)individuals,theprovisionofinitialandongoingtraining,andcompetencyassessmentsoftheseindividualsatspecifiedintervals.9.1(A3.1.1;A3.2.1;A3.2.3),9.2(4.3.1.1;4.3.2.1;4.3.3.1)2.2.1.Recordsofthequalifications,training,andcontinuingcompetenceofindividualsshallbemaintained.9.1(A3.1.3;A3.2.1;A3.2.3),9.2(4.3.1.2;4.3.3.1;4.3.4)3.0SpecimensN/A4.0MaterialsCompetencyassessmentchecklistsWrittenassessmentmaterialCompetencyAssessment/TrainingNeedsAssessmentandImplementationGuideGuideline:TG.001EffectiveDate:01June2000CompetencyCompaniontotheTechnicalResourceManualforHospitalTransfusionServicesTG.001Page2of65.0QualityControl5.1.Therecommendedfrequencyofassessmentisuponcompletionoforientation,annuallyandasrequiredforperformancemanagementoftechnologistswhorotatethroughorworkinthetransfusionmedicineareaofthelaboratory.5.2.Ideally,thetrainershouldbeatransfusionservicetechnologistwithsignificantexperienceandgoodcommunicationandfeedbackskills.Theemployershouldsupportandprovidecontinuingeducationininstructionalskillsandcommunications.5.3.Writtenassessmentmaterial,includingtheanswers,shouldbeavailableforallpersonneltoreviewasrequired,eitherindividuallyorwithatrainer.6.0Procedure6.1.Identifytrainingneeds.SeetheGuideforUseoftheTechnicalResourceManualor,alternately,identifyareasbyevaluatingexistingdocumentation.Thismayincludethefollowing.6.1.1.Initialtrainingandorientation6.1.2.Neworrevisedprocedures6.1.3.Errororincidentreportsinvolvingtestresultsorbloodproducts.6.1.4.Exceptionreportsorqualitycontroldocumentationindicatingatrendorshowingthataprocessoratestisfallingoutofcontrol.6.1.5.Documentationofspecificperformance-relatedissues.6.2.Selecttheguideline(s)thatrelatetothetrainingneed.Selectproceduresperformedbyalltechnologistsandanyspecifictrainingareas.Seeproceduralnotes8.1and8.2.CompetencyAssessment/TrainingNeedsAssessmentandImplementationGuideGuideline:TG.001EffectiveDate:01June2000CompetencyCompaniontotheTechnicalResourceManualforHospitalTransfusionServicesTG.001Page3of66.3.Obtainthecompetencyassessmentchecklistfortheguideline(s).Additionalfacility-specifickeyjobareasorassessmentcriteriarelatedtotheguidelinemaybewrittenontothechecklist.Ifthisisdone,everytraineeshouldbeassessedwiththesameadditionalcriteria.6.4.Fromthechecklist,determinetheprerequisiteactivities(ifany)requiredtofulfilthecompetency.6.5.Determinethetimerequiredforeachstaffmembertodemonstratetheprerequisiteactivities(ifapplicable)andrequiredcompetenciestothetrainer.Ifassessmentofmanyguidelinesisrequired,breakthetaskdownintosmaller“projects”thatinvolveuptoamaximumof5-6guidelines.6.6.Communicatetheplantostaff.Disseminateallmaterialsforreviewandcomment.Allowallstafftimetoabsorbtheinformationandtobeginthetask(seeTraineeInformation).Trytoallowenoughtimetoprepare,butsettimelinesforcompletion.6.7.Posttimelinesforstaff.Askstafftonotifythetrainerwhentheyarereadytoproceed.6.7.1.Allowtimeforstafftoself-assessusingthechecklist(s).6.7.2.Allowstaffaccesstothewrittenevaluationanswers,toencourageself-assessmentandlearningopportunity.6.8.Scheduletimeforthetrainerandtraineetocompletethedirectobservation.Thewrittenassessmentmaybereviewedbeforeorafterthedirectobservationoftheguideline.6.9.Recorddirectobservationresultsonthechecklist.Ifthetraineedoesnotperformoneofthestepsorifthetraineranswers“no”tooneofthequestionsinthe“KeyJobAreas/AssessmentCriteria”section,proceedtostep6.11.1.6.10.Ifallrowsonthechecklistareanswered“Yes”,placeacheckmarkinthe“Yes”boxbesidethe“Istraineecompetenttoperformprocedure”question.CompetencyAssessment/TrainingNeedsAssessmentandImplementationGuideGuideline:TG.001EffectiveDate:01June2000CompetencyCompaniontotheTechnicalResourceManualforHospitalTransfusionServicesTG.001Page4of66.11.Ifanyassessmentquestionsonthechecklistareanswered“No”,placeacheckmarkinthe“No”boxbesidethe“Istraineecompetenttoperformprocedure”question.6.11.1.Detailsoftheactionsthetraineeperformedinthedeficientkeyjobareashouldberecordedinthecommentsareaattheendofthechecklist.6.11.2.Aplanforremedialandcorrectiveactionshouldbewrittenonthechecklist.Thisshouldincludestepsthatthetraineemustcompletebef
