G318日喀则机场至日喀则市专用公路新改建工程A标段(K0+000-K26+500)架梁安全专项施工方案龙建路桥股份有限公司日喀则市至日喀则机场A标项目经理部-1-目录一、工程概况··········································································································二、编制依据··········································································································三、安全目标··········································································································四、安全管理组织机构······························································································五、安全生产保证体系······························································································六、作业人员岗位配备表···························································································七、安全教育培训····································································································八、安全施工保证措施······························································································九、资源配置··········································································································十、危险源辨识及应对措施························································································十一、建立架梁值班制度···························································································十二、实行安全生产联保互控制度···············································································十三、严格执行架梁作业纪律·····················································································十四、架梁作业安全技术操作规程的一般规定································································1.架梁现场安全操作规程·····························2.架梁现场人员的安全防护···························3.起重吊装安全操作规程····························4.高空作业安全操作规程····························十五、架梁作业安全操作规程·····················································································1.一般要求··································2.整机安全操作要求······························3.液压系统安全操作规程及注意事项·······················4.电气系统安全操作规程及注意事项·······················5.架梁前的准备工作······························6.架梁作业前安全注意事项···························7.提梁、运梁作业·······························8.架梁作业··································9.起吊机架梁及纵移过孔····························10.架梁作业重要部位安全操作规程························11.梁体安装的注意事项及安全保护措施······················-2-架梁施工安全专项方案一、工程概况本项目为西藏日喀则机场至日喀则市专用公路新改建工程。路线起点位于机场进场路与G318交叉处,路线终点在白朗县阿亚村,采用一级公路标准,设计速度100Km/h,路基宽度25m,路线全长26.5Km(不含断链)。主线起点至机场大门之间路段为机场进场路,采用市政道路标准建设,设计速度40Km/h,路基宽度22.46m/24.66m(既有道路单侧加宽)。白朗连接线采用二级公路标准建设,设计速度80Km/h,路基宽度12.0m,白朗连接线全长7.015Km。1、项目地理位置及地形、地貌本项目位于西藏日喀则地区,所在区域是青藏高原的重要组成部分。路线跨越年楚河,为雅江支流水系;主要翻越杰琼村垭口、崩戈虾日垭口和布苦努垭口,路线最高海拔位于崩戈虾日垭口标高4029米(K29+090),最低点位于路线起点3802.5m(K0+000),项目区大部分区域海拔位于3700-4100m之间。受构造控制,区内地貌按成因类型主要分为构造剥蚀地貌和河流侵蚀地貌两大类型。构造剥蚀地貌主要为藏西南中低山区,总体地形西高东低,而河流侵蚀地貌主要为雅鲁藏布江和年楚河及其支流的流水地貌。2、气候条件项目区气候空气稀薄、阳光充足、干燥缺氧、低温寒冷、夏秋多雨、冬春多风、年降水少、水热同步、年温差小而昼夜温差悬殊,干湿季节明显,夏季温和湿润,降水集中,冬季干燥寒冷、蒸发强烈为特征,属藏南温带半干旱高原季风气候。综合日喀则气象站、白朗江河办及西藏自治区地图册(1995年版)和西藏旅游探险手册(2001年版)等资料,测区内气压为549~624mmpa,空气密度为-3-719~800g/m3,含氧量为166~186s/m3,相对海平面含氧量约59%~65%。多年平均气温4.93~7.07℃,极端最高气温28.5℃,极端最低气温-16.8℃。多年平均降水量265.3~451.5mm,多年平均蒸发量为2114.9~2473mm,空气相对湿度43.44~45.73%,年日照时数为3190~3200h,年无霜期110---120天。中南部冻土深度小于0.93m。每年2月~4月气候干燥,常刮东南风或西北风,且全年盛行河谷风,平均风速1.62~3.3m/s,最大风速25m/s。5月~7月气候温和,一般地面温度15~20℃,月均气温为12.8~14.2°C,并以6月最高为13.9~16.4℃,7月~9月为雨季,季节分明,气候温和,多夜雨并呈大雨或暴雨,其降水量占年降水量的90.4~94.5%。12月至翌年1月为冬季,气候干燥寒冷,多积雪和冰冻,累年最冷月平均气温为-3.4~-6.4℃,为每年一月份,北部高寒山区气温更低。3、工程地质工作区沿线出露地层主要为第四系松散堆积层,下伏滇藏地层大地区之冈底斯—腾冲地层区南部日喀则分区及雅鲁藏布地层区北部蛇绿岩分区。其中日喀则地层分区主要出露白垩系日喀则群昂仁组(K2a)地层;而雅鲁藏布地区主要出露上侏罗~下白垩统白朗蛇绿岩群(J3K1B)及新生界古近系柳区群(E1-2Lq)地层。其中白朗蛇绿岩群变质、变形较强,为构造地层单位;柳群区为山前磨拉石沉积。二、编制依据(1)《龙门吊操作使用说明书》(2)《运梁小车操作使用说明书》-4-(3)《起重机械安全规程》(4)《公路工程施工安全技术规程》三、安全目标坚持贯彻“安全第一,预防为主,综合治理”的方针,建立健全安全管理组织机构,完善安全生产保证体系,消灭人身死亡、重伤、机具设备大事故等一切责任事故,创建安全生产标准工地。四、安全管理组织机构架梁安全管理组织机构-5-五、安全生产保证体系按照GB/T28001-2001健康安全管理体系标准的要求建立项目安全生产保证体系。领导小组组长:李玉生副组长:张凯现场总指挥:毕建伟副总指挥:张佳瑞现场技术人员:王登辉安全部长:郭越运梁作业队架梁作业队提梁作业队-6-安全保证体系框图专职安全检查员职工健康安全目标、指标无从业人员重伤,执行国家规定职业病防治。劳保用品发放覆盖率100%,使用率100%各种安全生产制度月季年监察制度实行安全生产承包责任制奖罚分明施工技术安全规程教育项目分部安质检查组提高全员安全意识项目部安全领导小组各项安全生产保证经济保证制度保证组织保证思想保证安全保证体系-7-六、作业人员岗位配备表七、安全教育培训1.组织厂家专业技术人员对架梁作业人员进行技术讲解,现场操作指导和演练。2.分阶段进行架梁作业的安全教育培训。在架梁作业开工之前,针对架梁作业的特点进行特殊工种、架梁工序、架梁岗位操作技能的培训;在架梁过程中,定期的进行安全再教育、再培训。序号班组岗位人数主要职责1总指挥1架梁施工作业总指挥2副指挥1发布指挥令、监督架梁施工3提梁队龙门吊司机及监控人员3负责龙门吊提梁的主控操作及监控4支座安装人员6负责预制梁吊装前的吊杆及支座安装5运梁队运梁小车司机及监控人员10负责运梁小车运梁的主控操作及监控6架梁队墩顶吊装工大里程墩柱吊装工16液压千斤顶操作,预制梁的就位安装及其他相关工作7小里程墩柱吊装工12液压千斤顶操作,预制梁的就位安装及其他相关工作8天车天车装吊工4主要负责天车的运行及钢丝绳的排绳状况的监控910机械维修组机械、电工2负责发电机组及设备的电气维修保养工作11测量组测量技术员8负责预制梁架设有关测量工作12技术组桥梁工程师2架桥施工技术工作13机械液压工程师1提、运、架设备技术工作14电气工程师1提、运、架设备技术工作15杂工2016材料工10材料联系供应17保安巡守4主要负责设备的看守及现场维护18汽车司机619合计109-8-3.由总工组织工程技术人员、安全工作人员,针对龙门吊提梁作业进行工程技术交底工作和安全技术交底工作。4.架梁开始前,对龙门吊司机、运梁车司机、电工、起重工等必须取得相应资格证书,做到持证上岗。对已取得上岗证的要进行存档登记,按期复审,名册应齐全,主要特殊工种作业人员必须佩戴上岗证。5.安全知识教育主要从基本生产概况、施工工艺方法、危险源及各类不安全因素和有关安全生产防护的基本知识入手。安全技能教育,结合各种专业特点,实施安全操作、规范操作的技能培训,使其熟悉掌握本工种安全操作技术;事故教育、法制教育,事故教育可以使其从事故教训中吸取有益的东西,可预防类似事故的发生,法制教育可以激发人们自觉地遵纪守法,杜绝各类违章指挥、违章作业行为。安全教育要求体现六性,即全员性、全面性、针对性以及成效性、发展性、经常性。6.要开展好各项安全生产活动,使安全生产警钟长鸣,防范于未然。同时还可以根据施工生产的特点实施好五抓的安全教育,即:工程突击赶任务时,工程接近收尾时,施工条件好时,季节气候变
