青年论文参赛论文基于IHEXDS架构的跨区域医疗信息共享交换系统架构设计与实现张凯，杨媛媛，凌彤辉,张建国中科院上海技术物理研究所医学影像信息学实验室200083摘要:针对病人在各医疗机构间就诊期间其医疗信息不能共享、无法进行协同医疗，本系统研究设计基于IHEXDS架构的跨区域医疗信息共享系统，并成功应用于国家科技支撑计划重点项目“基于IHE规范的大型城市医疗信息共享服务及示范工程”。系统设计遵循IHEXDS.b(Cross-EnterpriseDocumentSharing,v.b)和XCA(Cross-CommunityAccess)集成模式，采用SOA(ServiceOrientedArchitecture)和EDA(Event-DrivingArchitecture)技术架构进行实现。系统由四个层次组件组成：第一层:一个市级注册中心(Registry)；第二层:多个区级注册中心及相对应的多个文档存储池(Repository),每个区级注册中心对应相应的就诊管理域(DomainorCommunity)；第三层:提供病人基本信息的文档源(DocumentandImageSource);第四层:医疗信息使用者(Consumer)。本共享交换系统可以解决同一域内不同医疗机构之间的病人医疗信息共享，不同域之间病人医疗信息共享需求.系统采用医学图像网格技术控制和分发医学图像在同一域内以及不同域之间的共享交换应用，并可以实现病人在同一域内的不同医院之间转院及在不同域的医院之间的转院。采用LoaderRunner技术对设计研发的共享系统数据发布、用户查询和图像提取进行了压力测试评估。测试结果达到设计目标要求，因此本研究的基于SOA和网格技术而设计的区域共享交换系统能够有效的实现跨域的医疗电子文档的贡享与协作。关键字：IHEXDS.b,ElectronicHealthcareRecord,XACML,ebXML3.0,ATNA。Abstract:ShanghaiispilotingtodevelopanEHRsystemtosolvetheproblemsofmedicaldocumentsharingforcollaborativehealthcare,thesolutionofwhichisconsideringtofollowingIHEXDS(cross-enterprisedocumentsharing)andXCA(cross-communityaccess)technicalprofilesaswellascombinedwithgridstorageforimages.Thefirstphaseoftheprojecttargetstextandimagedocumentssharingcrossfourlocaldomainsorcommunities,eachofwhichconsistsofmultiplehospitals.Theprototypesystemwasdesignedanddevelopedwithservice-orientedarchitecture(SOA)andEvent-DrivenArchitecture(EDA),basingonIHEXDS.bandXCAprofiles,andconsistsoffourlevelcomponents:onecentralcityregistry;themultipledomainregistries,eachofwhichisforonelocaldomainorcommunity;themultiplerepositoriescorrespondingtomultiplelocaldomainregistries;andmultipledocumentsourceagents,eachofwhichislocatedineachhospitaltoprovidethepatienthealthcareinformation.Thesystemwasdevelopedandtestedforperformanceevaluationincludingdatapublication,userqueryandimageretrieval.TheresultsareextremelypositiveanddemonstratethatthedesignedEHRsolutionbasedonSOAwithgridconceptcanscaleeffectivelytoservemedicaldocumentsharingcross-domainorcommunityinalargecity.Keywords:IHEXDS.b,ElectronicHealthcareRecord,XACML,ebXML3.0,ATNA.
