HealthandSafetyRequirementsLafargeIliganInc.AnnexBPresentationnameorchapter2Date|AnnexBTHISSTANDARDFORHEALTH,SAFETYANDENVIRONMENTSHALLBEAPPLICABLETOALLLAFARGEASSOCIATEDCOMPANIES,INCLUDINGBUTNOTLIMITEDTOLAFARGECEMENTSERVICES,PHILIPPINES,INC.;REPUBLICCEMENTCORPORATION(BULACAN,NORZAGARAY,TERESA,andBATANGASPLANTS),LUZONCONTINENTALLANDCORPORATION,ILIGANCEMENTCORPORATIONANDMINDANOPORTLANDCEMENTCORPORATION.ALLAGREEMENTSENTEREDINTOBYTHESECOMPANIESSHALLBEGOVERNEDBYTHISDOCUMENT.INCASEOFCONFLICTBETWEENANYPROVISIONHEREUNDERONONEHAND,ANDANYPROVISIONINTHEMAINCONTRACTORANYPURCHASEORDERPURSUANTTOTHISCONTRACTONTHEOTHER,THELATTERSHALLPREVAIL.ALLCONTRACTORSHALLCOMPLYWITHTHEFOLLOWINGSAFETYPOLICYATALLTIMES.THESERULESARENOTINTENDEDTORESTRICTTHECONTRACTORORTORELIEVEHIMOFANYOFITSLEGALOBLIGATIONSBUTTOENSURESAFEWORKINGPROCEDURESORCONDITIONS.GENERALPROVISIONONHEALTH,SAFETYANDENVIRONMENTTheCONTRACTORshallappriseitselfoftheGroupHealthSafetyandENVIRONMENTStandards,RulesandRegulations,andAdvisoriesoftheCOMPANY.Intheabsenceofspecificstandards,thelocaland/orinternationalbestpracticeapplieswhicheverismorestringent.TheCONTRACTORshallensurethatitpossesstheregistrationandcertificationbytheGovernmentorregulatingbodysuchasDENR–MGBTheCONTRACTORshallensurethatpersonnelpossesswithTESDAorotherThirdPartyCertifyingBodyforwelding/cuttingworks,heavyequipmentoperatorandscaffoldingTheCONTRACTORshallundertakeapplicablesafetytrainingfortheirspecifictradefromCOMPANYaccreditedsafetytraininghouse(WSO,Insafety,SafetyHouse)-seeattachedPresentationnameorchapter3Date|Presentationnameorchapter4Date|TheCONTRACTORS’liftingequipment(gears,slings,shackle,crane,forklift,manlift,boomtruck)andheavyequipment(payloader,bobcat,bulldozer,backhoe,etc)shallhave3rdpartycertificationfromCOMPANYaccreditedcertifyingbody(TESDA,ICIP,1stPhilippines,WireRope,etc.)TheCONTRACTORshallassignatleastOne(1)RegisteredSafetyOfficerforevery50workersonsite,however,ifthecontractorisdoing2highriskjobsatthesametime,one(1)RegisteredSafetyOfficerisrequiredoneachjob.TheCONTRACTORmustconductRiskAssessmentofallworksandprovidesdetailsoftheenvironment,occupationalhealth(includesdrugtestingprogram)andsafetyimplicationsoftheworktobeundertaken,includingallrelevantMSDS.CONTRACTOR'spersonnelwhomaybeexposedtoparticularhazardsbyCOMPANYactivities,shallbeprovidedbytheCONTRACTORofadequateinstructionaboutthehazards(andifnecessary,trainingmustbeprovided)priortoundertakesuchwork.CONTRACTORmustensurethatallofitspersonnel:(1)possessedanuptodateclearances(NBI,Barangay,Police);(2)coveredwithsssandemployeescompensationandliabilityinsurance;and(3)hasvalidmedicalcertificate(bloodtest,chestx-ray,ECG35yo)withone(1)yearvalidityfromareputableCOMPANYaccreditedhospitalandorclinic.CONTRACTORsSiteSafetyCoordinatorisrequiredtoattendsafetymeetingeveryweekandsubmitaweeklysafetylistingatsecurity.Presentationnameorchapter5Date|SUBCONTRACTINGTheCONTRACTORmustobtainapprovalfromtheCOMPANY,priortoengagingasub-ContractorandtheCONTRACTORmustimposeuponthesub-contractortheCOMPANY’ssafetystandards,rulesandregulationsforCONTRACTOR.TheCONTRACTORmustensurethatitsSUB-CONTRACTORpossessalltheHealthandSafetyRegistrations,Certifications,Licenses,Training,andareadequatelyapprisedbytheCONTRACTOR.COMPLIANCEWITHLAFARGESAFETYPOLICYAPPLIEDONSITETheCONTRACTORanditsemployeesmustobserveandcomplyatalltimes,withtheCOMPANY’sPolicy.TheCOMPANYwillprovideallrelevantdocumentationtothateffectandshallexcludefromsiteanypersonnelwhobreachsuchrules.Presentationnameorchapter6Date|HealthandSafetyViolations1stoffence-orientationandwrittenexplanationbytheContractorSafetyinchargebeforeallowingtoworkagainplus1,500pesospenalty.2ndoffence-orientationandwrittenexplanationbytheContractorSafetyinchargebeforeallowingtoworkagainplus1,500pesospenalty3rdoffence-bannedtoworkfor1yearplusanother1,500pesospenaltytothecontractorListofOffencesPenaltygeneratingoffenceisnotlimitedtothefollowinglist.Intheeventwhereinapersoncommittedarelativelyunsafeactorbehaviourbutnotincludedinthelist.Presentationnameorchapter7Date|TypeAOffenceNotwearingmandatoryPPEuponenteringthePPEzone(safetyhelmetwithchinstrap,safetyglasses,HighVisibilityVestandsafetyshoes).NotusingspecifiedPPEindefinedareaasmarkedwithsafetysign(dustmask,earplug).Disobeyingorviolatingthespeedlimit,notwearingseatbelts¬wearingcrushhelmets.Notqualifiedpersonneloperatingcompanyequipmentsuchasservicevehicle(van),forklifttruck,payloaderetc.Driving/ridingmotorbikenotwearingcrashhelmet,notsecuringthecrashhelmetchinstrapandwithoutvaliddriver’slicense.Notwearingseatbeltwhiledriving/ridingavehicle(car,truck,forklift,etc)insidecompanypremises.Presentationnameorchapter8Date|TypeAOffenceNotusingpedestrianlane(walkingalongtheroad)insidetheplantarea.Notstoring/securinggascylindersproperly(cage,trolleyortiedwithrope/chain).Usingunsafetoolsandequipmentsuchasnotusingflashbackarrestor,damagedpressuregauges,unsafeladdersandfabricatedtools.Notwearingfaceshieldwhileusinggrinders,drillpress,lathemachineandoxy/acecuttingequipment.Equip
