目录总则························································································3第一部分应急救援预案·································································3第一章事故类型及危险源分析···················································31.危险源分析········································································32.危险源分布········································································4第二章事故应急救援·······························································41.应急救援基本原则·······························································42.应急救援组织机构及职责······················································52.1应急组织体系····································································52.2应急救援指挥中心及职责····················································5第三章报警与接警··································································91.响应级别确定···································································102.应急启动·········································································103.报警··············································································104.接警程序·········································································115.预警··············································································11第四章信息报告程序······························································11第五章控制与紧急公告···························································121.指挥与控制······································································122.报警和紧急公告································································13第二部分现场应急处置································································132第一章应急救援处置······························································131.响应分级·········································································132.响应程序·········································································14第二章现场处置措施······························································141.处理爆炸原则及要求··························································163.井下爆破灾害事故自救避灾方法···········································142.遇险人员现场处置·····························································16第三章应急结束····································································171.疏散与安置······································································172.应急结速·········································································173.善后处置·········································································17第四章保障措施与队伍演练·····················································181.保障措施·········································································182.演练···············································································183.应急预案备案···································································184.应急预案实施···································································185.制定与解释······································································183苏海图煤矿爆破灾害应急救援专项预案总则为了贯彻落实国家“安全第一、预防为主、综合治理”的方针,规范我矿爆破器材运输、仓储和使用安全工作,提高抗风险和防范事故的能力,特制定苏海图煤矿爆破器材运输、仓储和使用安全生产事故专项预案。第一部分应急救援预案第一章事故类型及危险源分析1.危险源分析依据《重大危险源辨识》原则,结合我矿爆破器材运输、仓储与使用的危险性分析。可能引发重大事故的危险源主要是:1.1.爆炸在采矿工程施工过程中,主要应用的煤矿许用乳化炸药和毫秒延期电雷管。由于广泛应用于煤矿工程爆破中,并且对周围介质特殊要求性,存在储存、运输和使用使用过程中由于各种原因导致突然发生爆炸,很容易引起周围介质着火和爆炸这些事故易造成人员群死群伤,财产重大损失。1.2.火灾由地面向井下运送火工品时,由于明火、受热自燃或撞击等原因,引起由弱到强燃烧,井下使用时,由于明火放炮、糊炮及瞎炮等原因引起火灾,甚至会引起其它介质燃烧或爆炸。1.3.中毒由于仓储不当,部分原材料受热分解后极能引起火灾,燃烧(爆炸)的产物中有大量氮化物和CO等有毒有害气体,造成人员中毒伤亡。1.4.失盗由于制度不健全,工作人员责任心不强,不按规章制度运输使用和清退,在部分环节上存在丢失爆炸物品。会给人民群众带来极大的安全隐患,从而影响社4会稳定。2.危险源分布由于我矿是一个老矿,煤层受地质条件影响,机械化程度较低,许多掘进工作面仍然以放炮落煤为主(改:虽然机械化程度较高,但在施工岩巷、材料道、峒室、溜煤眼等巷道时,仍然使用爆破方法。)因而爆破材料涉及我矿每个掘进单位。2.1.运输过程2.1.1.运输车辆不符合规定或矿车出现故障;2.1.2.车厢内危险品码放不符合规定,轨道质量不符合规定,因颠簸产生摩擦、碰撞导致事故;2.1.3.运输过程中,司机操作失误导致追尾、超速、落道、翻车,引发意外火灾、爆炸。2.2.储存过程2.2.1.搬运、装卸过程中操作失误;2.2.2.储存条件不符合规定,违反同库存放的原则;2.2.3.静电、射频、电气火化。2.3.使用过程2.3.1.工作人员在领用、清退过程中责任心不强,易发生火工品失盗现象;2.3.2.工作人员在使用操作过程中,没有认真执行“三大规程”导致操作失误;2.3.3.没有认真执行“三人连锁”井下爆破制度。第二章事故应急救援1.应急救援基本原则1.1.以人为本,安全第一。把保障人民群众的生命安全和身体健康、最大程度地预防和减少安全生产事故灾难造成的人员伤亡作为首要任务。切实加强应急救援人员的安全防护。充分发挥人的主观能动性,充分发挥专业救援力量的骨5干作用和人民群众的基础作用。1.2.统一领导,分级负责。在矿统一领导和组织协调下,各有关部门按照各自职责和权限,负责有关安全生产事故灾难的应急管理和应急处置工作。企业要认真履行安全生产责任主体的职责,建立安全生产应急预案和应急机制。1.3.依靠科学,依法规范。采用先进技术,充分发挥专业作用,实行科学民主决策。采用先进的救援装备和技术,增强应急救援能力。依法规范应急救援工作,确保应急预案的科学性、权威性和可操作性。1.4.预防为主,平战结合。贯彻落实“安全第一,预防为主,综合治理”的方针,坚持事故灾难应急与预防工作相结合。做好预防、预测、预警和预报工作,做好常态下的风险评估、物资储备、队伍建设、完善装备、预案演练等工作。2.应急救援组织机构及职责为迅速准确、有条不紊地进行救援抢险,按照我矿实际情况成立救援组织机构。本着专业对口分工、便于领导、便于集结和开展救援的原则,建立组织落实人员。2.1应急组织体系苏海图矿爆炸物品突发事故应急救援组织体系,以苏海图矿党政领导及有关部门、相关单位、公司救护大队及矿辅助救护队组成。应急救援组织体系图应急救援指挥中心救援抢险组技术支持组后勤救护组通讯联络组安全保卫组2.2应急救援指挥中心及职责为了便于在矿井发生灾害时,迅速有秩序救灾,及时地将灾害控制在最小范围,特成立抢险救灾应急指挥部,指挥中心设在矿调度室。指挥中心设总指挥、6副总指挥、值班室。总指挥:杨占彪副总指挥:张鹏飞、郭建平、钮长松、辛鹏飞、苏金亮谢春河、谢俊义、张寿林、路贵珍成员:杨茂林、项巨才、赵旭东、高耀军、刘福贵、史庆祥、崔永红、邬殿斌、王振江、贾卫东、梁玉忠、总指挥职责:总指挥是处理事故的全权指挥者,在副总指挥和个部门及部门所属各单位的协助下,制定抢险救灾和处理事故的救援计划;全面掌握救援情况,了解事故现场的最新动态,会同指挥中心成员根据现场情况拟定救援方案和措施;协调与外部应急力量、相关政府部门等关系,根据救援的进程,决定是否申请其他救护力量提供援助。副总指挥职责:负责制定抢险救灾、处理事故的救援计划。根据批准的应急救援计划,对救援过程的人员安全实行有效的监督。根据抢险救灾和处理事故的需要,负责组织人员,及时调集救灾所必须的设备、材料,联系组织医疗人员、医疗设备及信息的发布。值班室职责:1)负责通知指挥中心成员到中心报到,将事故情况报告总指挥,传达指挥中心的命令;随时
