内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-0-申报书校团委:兹报送2014年暑期“三下乡”社会实践活动申报书一式一份,请审核。申报单位数学与信息科学学院申报团队“筑梦北川”志愿者调研服务队指导教师张莲莲联系方式13619008528负责人邹昀联系方式15708322512标书上交日期:2014年5月26日内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-1-目录申报表···············································4队伍宣传单···········································12志愿服务队活动方案···································13活动背景及主题·······································14必要性分析···········································14可行性分析···········································16预期效果·············································16活动意义·············································17义务服务流程安排·····································17活动经费预算·········································18附件一···············································20附件二···············································21附件三···············································23附件四···············································25附件五···············································26附件六···············································27附件七···············································28附件八···············································29附件九···············································31附件十···············································32附件十一·············································34内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-2-附件十二·············································36附件十三·············································38附件十四·············································39附件十五·············································40附件十六·············································41志愿服务调研队活动方案·······························42活动主题及调研内容···································43必要性分析···········································44可行性分析···········································45预期效果·············································45活动意义·············································46调研流程安排·········································46活动经费预算·········································47附件一···············································48附件二···············································49附件三···············································51附件四···············································52附件五···············································53附件六···············································54附件七···············································56内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-3-附件八···············································57附件九···············································58附件十···············································60附件十一·············································61附件十二·············································62内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-4-内江师范学院2014年暑期“三下乡”社会实践活动校级重点团队申报表团队名称“筑梦北川”志愿者调研服务队申报单位数学与信息科学学院指导老师张莲莲实践类型义务支教、关爱留守儿童或“三孤”人员、科技服务、知识培训、文艺演出、专题考察、参观访问。团队负责人姓名邹昀性别女学院班级数信学院2012级3班政治面貌入党积极分子联系方式15708322512Email:695716813@qq.com活动时间2014年7月7—7月21日活动地点绵阳市北川县开坪乡经费预算1、队员短期临时保险10元/人*14人=140元2、调研及资料用品:300元3、问卷及宣传单:300元4、综合素质活动用品:300元5、文艺所需经费:400元6、走访敬老院、贫困家庭礼品:400元7、药品:210元总计:2050元活动简介(活动内容、行程安排、活动意义等)活动目的、主题、内容内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-5-主题:义务支教,汉羌同成长;研究红豆,科技系丰收;震后对比,感党之伟大;公益宣传,懂抗震知识;队伍宣传,传播内师情;文艺汇演,谢八方支援;大学生活,青春展年华;地震演习,熟紧急避难。志愿服务内容:1、义务支教,汉羌同成长结合队员自身的特点,以数学、物理、心理健康为主要科目,兼顾学生们的需要以及兴趣爱好,对北川及周边乡镇的小孩进行义务支教志愿服务。2、队伍宣传,传播内师情通过宣传单、PPT、《青春年华》校刊,给当地学生、老师、家长展现内江师范学院校园环境、教学设施、学生活动风采等。让老师和家长了解内江师范学院;让学生对多彩的大学生活有一定的初步认知,明确学习的目标并为之不断奋斗。3、文艺汇演,谢八方支援灾难无情,人间有爱。一方有难,八方支援。社会各界的爱心暖流正源源不断的汇集在了芦山这个小城镇里,让受到伤害的受灾同胞们不再感到孤寂和冷漠。4、公益宣传,懂抗震知识因为北川处于处于我国一个大地震带--南北地震带上,所以普及避灾自救互救知识十分重要。5、大学生活,青春展年华农村的孩子们对大学生活了解甚少,我义务服务小分队将通过播放视频的方式为孩子们展现我们的丰富多彩的大学生活,同时解决孩子们的困惑,引导孩子们努力学习,明确自己的人生目标。6、地震演习,熟紧急避难。北川处于大地震带上,紧急疏散演习活动开展地非常有意义,不但增强了同学们应付突发事情的意识。二、调研内容:1、研究红豆,科技系丰收红豆杉是世界上公认濒临灭绝的天然珍稀抗癌植物,红豆杉的经济价值,药用价值很高,但是不易培养。我们将运用所学知识,构建数学模型,建立合理的种植及运送方式。2、震后对比,感党之伟大。本次活动通过对具有代表性的当地人民进行问卷调查,了解震后重建工作。党的领导核心作用得到了充分发挥。3、农村及少数民族受教育情况问卷调查少数民族地区教育发展是关乎民族地区持续快速发展的大问题。本次活动通过对具有代表性的当地少数民族地区进行问卷调查,旨在了解当地家长对教育问题的看法和意见,提高对教育的重视程度。4、生活环境及生活习俗问卷调查通过进行对当地少数民族的生活环境、生活习俗的问卷调查,了解当地少数民族的一些生活习惯,并从科学的角度为他们提供一些生活技巧与养生保健知识。必要性分析内江师范学院数学与信息科学学院“筑梦北川”志愿者调研服务队-6-1.北川县位处四川省西北部,其处少数民族人数较多。北川县位于四川省西北部,东接江油市,南邻安县,西靠茂县,北抵松潘、平武县,有羌、藏、回、苗、壮、土家、满、黎、维吾尔、基诺、达斡尔等少数民族,其中,少数民族人口81370人,占全县总人口的50.5%,其中羌族人口77240人,占少数民族总人口的94.9%。不管是经济还是文化发展,都要比很多城市慢,所以,到当地进行科技文化宣传是很有必要,很有意义的。2.北川经济在整个四川属于中下水平,所以对其调查具有一般性质。经济的发展影响着农民的生活状态和生活理念,我们致力于了解整个四川的农村情况,可是条件有限不能到每一个地方去做实际的调查,因此选择一个具有一般性的地方详细的分析有助于我们把握整个四川农村发展的态势。3.北川县作为革命老区,具有光荣的革命传统。北川人民具有光荣的革命传统,1935年4月,中国工农红军第四方面军长征进入北川县境,在北川县相继建立了两个县级苏维埃、5个区苏维埃、28个乡苏维埃,119个村苏维埃政权。北川县人民筹粮运粮,修路架桥,男女青年踊跃参军。7月,红军离境,接踵而至的川军烧杀抢掠,无恶不作,杀人如麻,全县人口减少近半。北川人民为革命做出了巨大牺牲。1953年11月,四川省革命老根据地建设委员会将北川确定为“革命老根据地”。通过此次调研,有助于当代大学生了解历史,弘扬革命精神,继承光荣的革命传统。4.大学生有义务有责任走进农村,体验或回味农村生活。改革开放以来,中国的经济发展突飞猛进。为这个对社会发展做出极大贡献的群体一直是社会的焦点,这个群体就是生活在农村的广大劳动人民。当代大学生能够享有如此美好的生活,在一定程度上是与这个群体有着密切关系的。所以,作为新社会新世纪的优秀大学生更应该具备积极向上、不怕吃苦、勇于奉献,以实际的行动为农村居民服务。5.作为师范院校的大学生,义务支教是一种必不可少的志愿服务。农村和城市的学校在硬件设备、师资力量等方面均存在一定的差异,特别是少数民族地区的农村,差异更加的大。这种差异直接影响城乡学子所接受的教育方式和教育理念。同时,少数民族地区的农村学校在很大程度上忽视了对学生的心理健康教育与素质培养。所以,作为师范院校的大学生更应该走进农村,关心农村教育,在农村展开义务支教志愿服务活动,训练学生逻辑思维与激发学生学习兴趣。6.城乡儿童接受的教育方式和享有的教学设施存在巨大差距。随着城乡经
