清远市清城区东城新区二期海逸路工程工程现浇箱涵工程模板与支架专项施工方案-2-目录一、工程概况············································································································3二、施工组织和管理机构························································································3三、箱涵涵身施工工艺流程····················································································4四、施工进度计划····································································································5五、箱涵涵身施工要点····························································································5六、扣件钢管顶板模板支架计算············································································6七、腹板(墙)模板计算························································································7八、施工技术要求····································································································7九、安全管理···········································································································12-3-十、确保工程质量的措施·······················································································15十一、文明施工·······································································································16十二、预防伤害安全生产事故应急救援预案························································17十三、附件················································································································23附件1·························································································································23附件2·························································································································29附件3·························································································································35-4-一、工程概况清远市清城区东城新区二期海逸路工程程共有箱涵1座,本箱涵为4.0×4.0m双孔箱涵,中心里程桩号K0+550,箱涵中心线与设计混合车道中心线正交,涵长16.6m。采用整体式钢筋混凝土涵身,扶壁式挡土墙翼墙。箱涵净孔尺寸为4.0×4.0m,箱涵宽9.2m,长13.2m。顶板厚40cm,底板厚40cm,中腹板厚40cm,边腹板厚40cm,涵身为现浇C30混凝土。底板下缘浇注10cmC15混凝土垫层,其下为10cm碎石垫层。二、施工组织和管理机构项目经理:项目副经理:项目技术负责人:测量工程师:材料员:安全员:资料员:质检工程师:钢筋班组其他综合班组模板班组混凝土班组项目负责人:-5-三、箱涵涵身施工工艺流程-6-四、施工进度计划碎石垫层施工测量放样浇注垫层砼绑扎底板及腹板30cm钢筋钢筋制作钢筋检验安装模板浇注底板及腹板30cm砼模板拆除砼养护搭设顶板支架和模板绑扎腹板及顶板钢筋安装腹板模板浇筑顶板和腹板砼拆除模板和支架砼养护-7-见附件3。五、箱涵涵身施工要点1、编制依据①施工图设计文件及地勘报告,以及设计变更、补充、修改图纸及文件资料。②国家有关的政策、法规、施工验收规范和工程建设标准强制性条文(城市建设部分),以及现行有关施工技术规范、标准等。③《公路桥涵设计规范》;④《公路桥涵施工技术规范》;⑤《公路桥涵施工手册》;⑥《公路桥涵钢结构及木结构设计规范》JTJ025-86;⑦《建筑施工脚手架实用手册》;⑧《建筑施工模板安全技术规范》JGJ162-2008;⑨《建筑施工扣件式脚手架安全技术规范》JGJ130-2001。2、测量放样涵洞测量放样时,应注意核对涵洞纵横轴线的地形剖面图是否与设计图相符,应注意涵洞长度、涵底标高的正确性。对斜交涵洞、曲线上和陡坡上涵洞,应考虑交角、加宽、超高和纵坡对涵洞具体位置、尺寸的影响,并注意锥坡、翼墙、一字墙和涵洞墙身顶部和上下游调治构造物的位置、方向、长度、高度、坡度,使之符合技术要求。3、15cm碎石垫层15cm碎石垫层采用级配良好的碎石,含泥量符合设计要求,压实度要满足设计要求。4、C15砼垫层15cm碎石垫层经监理验收合格后浇注垫层砼,垫层砼采用商品砼,商品砼采用搅拌车运至现场模浇注,振捣密实,采用覆盖土工布浇水保湿养护;5、箱涵涵身施工箱涵涵身竖向施工施工分2个阶段,第一阶段浇筑至底板内壁以上30cm;第二阶段浇筑剩余部分,2次浇筑的接缝应保证有良好的衔接面(粗糙、干净并不得有堆落的混凝土和砂浆)。(1)模板安装前,应由测量放出承台和腹板的边线和中轴线,并用墨斗弹出边、轴墨线,-8-放样后经监理检查,合格后安装模板;(2)箱涵顶板底模、腹板侧模采用12mm、15mm厚木胶合板做面板,箱涵腹板模板支撑体系内楞采用5cm×10cm方木,间距20cm、外楞采用Φ48×3.0mm及Φ14对拉螺栓,间距40×40cm。,采用止水型对拉螺栓平衡侧模板受力。机械配合人工进行模板安拆;(3)现浇箱涵顶板支架采用扣件式钢管满堂支架,钢管采用φ48×3.0mm,模板支架搭设高度为4.0m,立杆的纵距b=0.60m,立杆的横距l=0.60m,立杆的步距h=1.20m。纵、横向杆及立杆用扣件式钢管连接,支架纵横均按有关规范要求设置剪刀撑,竖向剪刀撑横桥向在两端和中间各设一道,纵桥向两端各设一道,水平剪刀撑设每层水平杆各设一道。距离地面不超过20cm设置纵横扫地杆。支架立杆顶设二层肋梁:下肋梁为横向φ48×3.0mm钢管,间距60cm;上肋梁为纵向方木5×10cm,间距25cm。(4)在箱涵四周设置双排脚手架,采用单立杆,搭设高度为6.2m,立杆的纵距1.20米,立杆的横距0.8米,内排架距离结构0.30米,立杆的步距1.20米。钢管类型为48×3.0,脚手板采用竹笆片,栏杆采用竹笆片,脚手板下大横杆在小横杆上面。地基为粘土地基,压实后满铺5cm的木板。钢管顶端安放可调支座,纵向受力杆安放在可调槽上,两边用木塞契紧,其上安放横向5×10cm方木,间距25cm,顶层满铺12mm厚木胶合板。(5)内侧模及外侧模板应在同步养护的砼试块强度达75%时方可拆除,箱梁底模及支架必须在砼强度达到设计强度的100%后方可拆除。支架、侧模和底模采用人工拆除。拆除时模板和支架分类对放整齐。(6)箱涵普通钢筋的施工严格按有关技术规范认真施工,材料进场后经严格检验合格后加工下料、制作和绑扎。(7)现浇箱涵砼采用商品砼,用搅拌车运至现场浇筑。(8)箱涵砼浇筑完毕,表面收浆后即开始对砼洒水养生,养护采用覆盖土工布洒水保湿的养护方法,养护时间不少于14天。六、扣件钢管顶板模板支架计算见附件1,由PKPM专业软件计算所得七、腹板(墙)模板计算-9-见附件2,由PKPM专业软件计算所得。八、施工技术要求1、模板安装(1)模板安装前必须做好下列安全技术准备工作:①应审查模板结构设计与施工说明书中的荷载、计算方法、节点构造和安全措施,设计审批手续应齐全。②应进行全面的安全技术交底,操作班组应熟悉设计与施工说明书,并应做好模板安装作业的分工准备③应对模板和配件进行挑选、检测,不合格者应剔除,并应运至工地指定地点堆放。④备齐操作所需的一切安全防护设施和器具。(2)模板安装构造应遵守下列规定:①模板安装应按设计与施工说明书顺序拼装。②竖向模板和支架立杆支承部分安装在基土上时,应加设垫板,垫板应有足够强度和支承面积,且应中心承载。基土应坚实,并应有排水措施。对湿陷性黄土应有防水措施;对特别重要的结构工程可采用混凝土、打桩等措施防止支架柱下沉。对冻胀性土应有防冻融措施。③模板及其支架在安装过程中,必须设置有效防倾覆的临时固定设施。④现浇钢筋混凝土梁、板,当跨度大于4m时,模板应起拱;当设计无具体要求时,起拱高度宜为全跨长度的1/1000~3/1000。(3)安装模板应保证工程结构和构件各部分形状、尺寸和相互位置的正确,构造应符合模板设计要求。模板应具有足够的承载能力、刚度和稳定性,应能可靠承受新浇混凝土自重和侧压力以及施工过程中所产生的荷载。(4)拼装高度为2m以上的竖向模板,不得站在下层模板上拼装上层模板。安装过程中应设置临时固定设施。(5)支撑板的支架立杆安装构造应符合下列规定:①梁和板的立杆,纵横向间距应相等或成倍数。②在立杆底距地面不超过200mm高处,沿纵横水平方向应按纵上横下的程序设扫地杆。(6)施工时,在已安装好的模板上的实际荷载不得超过设计值。已承受荷载的支架和附件,不得随意拆除或移动。-10-(7)安装模板时,安装所需各种配件应置于工具箱或工具袋内,严禁散放在模板或脚手板上;安装所用工具应系挂在作业人员身上或置于所配带的工具袋中,不得掉落。(8)当模板安装高度超过3.0m时,必须搭设脚手架,除操作人员外,脚手架下不得站其他人。(9)吊运模板时,必须符合下列规定:①作业前应检查绳索、卡具、模板上的吊环,必须完整有效,在升降过程中应设专人指挥,统一信号,密切配合。②吊运散装模板时,必须码放整齐,待捆绑牢固后方可起吊。③严禁起重机在架空输电线路下面工作。④5级风及其以上应停止一切吊运作业。(10)木料应堆放于下风向,离火源不得小于30m,且料场四周应设置灭火器材。(11)墙模板应符合下列规定:①当用散拼定型模板支模时,应自下而上进行,必须在下一层模板全部紧固后,方可进行上一层安装。当下层不能独立安设支撑件时,应采取临时固定措施。②当采用预拼装的大块墙模板进行支模安装时,严禁同时起吊两块模板,并应边就位、边校正、边连接,固定后方可摘钩。③模板未安装对拉螺栓前,板面应向后倾一定角度。安装过程应随时拆换支撑或增加支撑。④对拉螺栓与墙模板应垂直,松紧应一致,墙厚尺寸应正确。⑤墙模板内外支撑必须坚固、可靠,应确保模板的整体稳定。当墙模板外面无法设置支撑
